Unique is a charity and is almost entirely funded
by voluntary donations. If you wish, you can donate to Unique in Pounds
Sterling, U.S. dollars,
Euros, Australian dollars, by international money
order in your own currency or by any of the major credit
cards. Donations should be made payable to Unique

Please print this, fill in your own account details and the amount you wish to donate and send it to
Julie Griffin at the address quoted above. The form includes a Gift Aid declaration
if you are a UK taxpayer.
Unfortunately the form
cannot be completed online as your bank requires your signature
to confirm that you wish to set up a new standing order payment and we require your
signature as confirmation that you accept the terms of Gift Aid.
Please remember that you can cancel the arrangement at any time by writing to your bank.

Please note that under the Data Protection Act, we are legally obliged to
inform you that the details of your donation (amount and purpose) are held on
a separate, secure Unique database and used only for the following reasons:
1) to retain accurate records for the purpose of claiming Gift Aid from the Inland Revenue
2) to inform you on a regular basis (usually annually) of the financial performance of the charity.
Your details are not passed on to any third parties. You may access this information at
any time by writing to us.
