Unique Members Area Registration

The Unique Members Area is intended to provide Unique members with a place to share information. Registering with the Unique Members Area enables you to read messages posted by other members and to post messages which can be read by other members. It also gives you access to the Unique Newsletter archive and the Unique Rare Chromosome Disorders Information Leaflets download.

If you wish, you can register with the Unique Members Area by completing the form below and pressing the button marked "Register". Only Unique members can register. You will require your 6 character Unique membership number which can be found on the address label of the Unique newsletter. You will also need to enter the first four letters of the family name you have registered with Unique. Each family can have up to 6 registered online names. To prevent misuse of the forum, please do not disclose your membership or login details to anyone else.

Choose any online name and password you wish between 6 and 12 characters long. If your family name contains an apostrophe, a space or a hyphen please omit it when you enter the first 4 characters of your name (e.g. O'Donnell becomes ODON, Van Betuw becomes VANB, Lee-Philips becomes LEEP). Please make a note of your chosen online name and password because you will need them to logon in the future.

Enter your Unique Membership Id	all 6 characters
Enter the first 4 characters of your registered family name	omit apostrophes, spaces and hyphens
Enter your chosen online name	between 6 and 16 characters
Enter your chosen Password	between 6 and 12 letters and digits only
Confirm your chosen Password	must be identical to your chosen password
Enter your Email Address	(optional)

Please complete the above form and press the button marked "Register" to start the registration process or press Reset to clear the form. Make sure you have made a note of your password because you will need it to login later.
